What percentage of medical claims do insurance companies reject?  Since, due to the coding medical claim billing is somewhat complex, this question is answered in two parts.  It has been seen that insurance companies reject close to 65% when the claims are sent for the first time.  The rejection rate for second time rejection comes at approximately 5-10%.  This ideally means that doctors would get paid at the most after the second time the claims are sent.  After careful analysis of the scenario, it has been deduced that these high rates of rejection are due to the lack of understanding of the codes by the coders.
